Personal data subjects (hereinafter referred to as “data subjects”), who are defined as data subjects in the Law No. 6698 on the Protection of Personal Data (“Law”), are granted certain rights regarding the processing of their personal data in Article 11 of the Law.

Pursuant to paragraph 1 of Article 13 of the Law; applications to be made to our Company, which is the data controller, regarding these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (“Board”). In this context, the applications to be made to our Company in “written” form by using this form;

By registered letter with return receipt,
Through a notary public,
Signing this form with the “secure electronic signature” defined in the Electronic Signature Law No. 5070 and sending it to the registered e-mail address of our Company,
By filling out this form and applying to the relevant facility in person,

The above-mentioned channels are “written” application channels pursuant to paragraph 1 of Article 13 of the Law. In addition, our Company will announce how the applications will be received by filling out this form and applying to the relevant facility in person and other methods to be determined by the Board.

Pursuant to paragraph 2 of Article 13 of the Law, your applications submitted to us will be responded within 30 (thirty) days from the date your request is received by our Company, depending on the nature of the request. Our responses will be delivered to you in writing or electronically in accordance with your choice in this application form in accordance with the provision of Article 13 of the Law.

The information and documents requested from you according to the nature of this form and your request must be provided to us completely and accurately. If the requested information and documents are not provided as required, there may be problems in the full and qualified execution of the researches to be carried out by our Company based on your request. In case the information and documents requested by our Company in this application form are missing, you will be contacted and requested to provide this information and documents to our Company as soon as possible. Until such information and documents are duly provided to our Company, the application evaluation period of 30 days will be suspended. In this case, our Company reserves its legal rights. For this reason, the relevant form must be sent completely and in a manner to include the requested information and documents according to the nature of your request.

This application form has been issued in order to determine your relationship with our Company, to determine your personal data processed by our Company, if any, and to respond to your application correctly and within the legal period. In order to eliminate the legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, our Company may require additional documents and information (identity card or

driver’s license copy etc.) reserves the right to request. In the event that the information regarding your requests submitted within the scope of the form is not correct and up-to-date or an unauthorized application is made, our Company does not accept any liability for the requests arising from such incorrect information or unauthorized application. For this reason, the relevant form must be submitted completely and in a manner to include the requested information and documents according to the nature of your request.
